This section of our website provides access to all guidance booklets and forms you may require from time to time.

You can obtain forms and guidance on:

  • Making a Lasting Power of Attorney (LPA)
    Here you can download a pack containing the forms and guidance you need to make either a finance and property or personal welfare LPA.
  • Registering a Lasting or Enduring Power of Attorney (EPA)
    Here you can download a pack containing the forms and guidance you will need to register either an LPA or EPA.
  • Asking the Court to make a decision
    Here you can find a link to the HMCS page containing the forms and guidance you will need to make the most common applications to the Court of Protection (the Court).
  • Additional publications and newsletters
    Here you can download booklets that provide you information on the OPG service standards and fees, remissions and exemptions, how to search the OPG registers and how to complain. Also guides for those people who have been appointed as Deputies, a guide on Supervision and newsletters such as Reaching Out and the MCA update.

    Document types provided

    Most of the documents we provide are in the Adobe PDF format.

    Where several documents are required to be read and completed we make them all available in an application pack for your convenience. These packs are packaged as .ZIP files.

     PDF Documents

    In order to view and print Adobe PDF documents you will need to have a copy of the free viewing software, Adobe Acrobat Reader, installed on your computer. If you do not have a copy then you can download Link to external website Adobe Acrobat Reader. Our page on help with accessing Portable Document Format (PDF) files provides more information.

     Microsoft Word

    In order to view, complete and then print the Microsoft Word documents you will need to have a copy of the Microsoft Word application installed on your computer.

    However if you do not have a copy of the application you can still view and print a Microsoft Word document using a copy of the free Microsoft Word viewer. You should have this viewer installed on you computer before you download one of our word forms. A copy of the free viewer is available for download from the Link to external website Microsoft website.

    Please note: The Microsoft Word viewer will only allow you to view and print word documents, you will not be able to complete any of the forms electronically without having a copy of the Microsoft Word application installed on your computer.

     ZIP files

    Documents are also packaged as ZIP files so that multiple related publications are grouped together for your convenience.

    Most modern computer operating systems should allow you to open this type of file. However if you have any problems opening ZIP files then then please contact Customer Services for further advice.
